What you need to do:

Identify all automated debits (ACH) and/or Direct Deposits you wish to switch to your new Firstrust account.

Fill out the forms below.
Switch Kit forms:
Direct Deposit Authorization Form - Use this form to establish Direct Deposit of your payroll check. Complete this form, sign it and send it to your payroll department.
Automatic Payment Request Form - Use this form to notify any service providers of your bank change so all future payments will be made from your new Firstrust account!
Account Closure Request Form - Use this form to notify your financial institution of the accounts you are closing. Complete the form, sign it and send it to that financial institution.
Helpful Hints:

Upon the first visit to your new Firstrust branch, please bring two (2) forms of identification with you. We will ask you to sign a signature card and copy your identification to complete the process.

Use your most recent bank statement to gather any automatic payments or deductions.

Make sure you allow time for any outstanding debits and/or payments to clear. It usually takes up to 10 days for any outstanding transactions to clear.

To complete the switch kit forms above, you will need your Firstrust Checking account number and the Firstrust Bank Routing number which can be found on your new checks. View a sample check.

Direct Deposits are recurring payments automatically deposited into your account on a regular basis. Examples are payroll, social security, alimony, dividend or disability payments.

Automatic Payments are recurring payments automatically withdrawn from your account on a regular basis. Examples include mortgage payments, gym membership fees, electric bills, or insurance premiums.
